Hours of Work: 37.5 hours per week - Monday - Friday 09:00-17:00 with some flexibility

Purpose of Role
To support a cohort of clients with varying degrees of complex and housing needs whose risks exceed what traditional supported accommodation models offer. Your caseload will be a hybrid of clients in the community and in a supported housing setting. The support will not create or increase dependence, it is “do with” rather than “do for” so it is anticipated that you will accompany service users to appointments, meetings and other activities initially. Support for this service could include the provision of advice, support, information, help and onward advice with regards to housing, or homelessness, welfare benefits, healthcare, debt, family and relationships and safeguarding. This role could involve referrals to MARAC and MDT/safeguarding meetings.
This role is subject to an Enhanced DBS Disclosure.
Please note: Driving Licence and use of a vehicle is Essential
