Hours of Work:  22.5 hours per week -  working across three day shifts, specific days to be agreed upon successful appointment. There will be flexibility needed to cover weekends and evenings as the need arises and you will work one week on call with a telephone service approximately every 6 weeks. Additional payment of £135 for the on-call week.

Project Description

The project operates across Pembrokeshire, identifying and working with individuals that are sleeping rough. The service operates flexibly across 7 days offering humanitarian aid in the form of food provision, issue of sleeping bags, warm clothing and toiletries and by helping to resolve issues, such as housing, healthcare, financial and substance misuse, by offering advice, guidance through a range of support and referrals to relevant specialist services. The service will be especially beneficial for clients who are, leading extremely chaotic lifestyles as they are able to meet and engage with Assertive Outreach Workers on a regular basis.

Purpose of Role

The post will engage with Service Users who are Street homeless or sleeping in buildings not designed for habitation.

To proactively provide outreach support and interventions to People who are street homeless in an assertive manner to secure meaningful engagement bringing about sustained and improved outcomes. To coordinate and provide support that enables and empowers Service Users to address housing and/or personal difficulties, build their skills and confidence, and ultimately improve their ability to access and maintain accommodation.  Support will help people improve their control, understanding and involvement in dealing with the issues that affect them, and assist Service Users to achieve their preferred outcomes.

This role is subject to an Enhanced DBS disclosure.
