Project Description
The project operates in Torfaen, working with people who may be homeless, potentially homeless or vulnerably housed. The service provides housing related support within people’s homes or in the community, with the aim of preventing homelessness and increasing independence.
Purpose of Role
To support homeless prison leavers prior to, during and post release to ensure they have a seamless transition from prison to resettlement back into the community.
The majority of the work will involve working with clients in prison so that housing and support needs can be identified and planned for through partnership working with the Local Authority and other agreed partners. A key element to the role following release will be to ensure a planned and coordinated handover to other agencies takes place.
This role is subject to an Enhanced DBS disclosure.
Please note: Driving Licence and use of a vehicle is Essential
